Enhanced Case Manger
The Enhanced Case Manager is responsible for delivering comprehensive care management services and providing community-based care coordination to address service gaps experienced by Medi-Cal recipients. This role focuses on optimizing health outcomes, improving access to services, and promoting stability for individuals facing medical, behavioral health, and social challenges. Roles and Responsibilities
Direct Care and Case Management
Provide culturally competent services to all participants, ensuring inclusivity regardless of race, color, creed, national origin, marital status, sexual orientation, age, sex, religion, disability, or payment status.
Conduct outreach efforts to engage and enroll eligible participants in the Enhanced Case Management (ECM) program.
Perform comprehensive assessments to identify medical, behavioral health, and social service needs.
Develop and implement individualized care plans, setting goals and tracking participant progress.
Provide education on self-management techniques and address barriers to accessing healthcare and social services.
Ensure participants are informed of their rights, responsibilities, and grievance procedures.
Collaborate with interdisciplinary team members to coordinate care and optimize client outcomes.
Uphold ethical and professional standards, including client confidentiality and cultural sensitivity.
Educate participants, families, agencies, and the community on health and mental health care issues, including prevention and early intervention.
Provide safety planning and guidance for participants with physical and cognitive challenges.
As needed, facilitate transportation for participants to medical appointments, social service meetings, or other essential activities.
Teamwork and Collaboration
Maintain professional and cooperative working relationships with colleagues, service providers, and community organizations.
Participate in regular team meetings and training to enhance service delivery and program effectiveness.
Work collaboratively to meet program goals and outcome expectations.
Provide feedback to management on program effectiveness, quality improvements, and participant satisfaction.
Reporting and Documentation
Ensure compliance with mandatory reporting requirements, including suspected abuse of dependent adults and elders.
Maintain clear, accurate, and professional records, ensuring all documentation meets regulatory and agency standards.
Meet caseload expectations while ensuring high-quality service delivery.
Submit timely and accurate billing data, invoices, and other required reports.
Qualifications
Bachelor’s degree in social work, psychology, or a related field OR
Three years of direct service experience in outreach programs, case management, or working with individuals facing housing instability, substance use, or other social service needs.
Knowledge and Skills
Experience assisting with housing navigation, community outreach, and direct service provision.
Experience working with the Cal Aim program
Understanding of HIPAA, OSHA, and all relevant state and local laws regarding healthcare and social services.
Knowledge of trauma-informed care principles and their application in case management.
Ability to read, speak, write, and comprehend English proficiently to communicate regulations, service requirements, and participant needs effectively.
Strong presentation skills and ability to respond to participant and stakeholder inquiries.
Basic mathematical skills to calculate service-related figures such as billing, budget allocations, and service tracking.
Strong problem-solving skills, including the ability to address complex participant needs and adapt to changing situations.
Other Requirements
Ability to complete tasks in a timely manner, meet deadlines, and effectively coordinate multiple activities.
Proficiency in using email, word processing, and case management software.
Commitment to maintaining professional and cooperative working relationships with colleagues and community partners.
